Apology: the editing process for the report was complicated and laborious. In order to ensure anonymity we went through a process of changing all names, classifications and workplaces in the report wherever possible. Following this process, we redacted references to workplaces to ensure that potentially damaging information was not wrongly attributed. It seems that one such reference was not redacted. Page 17 of the report makes specific mention to Western Hospital. The quote attributed to ‘Gerry’ relates to another hospital and not to Western Hospital. The VAHPA apologises to Western Hospital on the basis that this quote has the potential to create a false impression of work practices at that hospital.
